Immunization Requirements
Students enrolling in school must present verification that they have received the following
immunizations:
Kindergarten, T-1, 1st, 2nd, 3rd, 4th, 5th and 6th grade students will need:
2 MMR (both must be given on or after the 1st birthday)
5 DPT (unless the 4th shot was given on or after the 4th birthday)
4 Polio (unless the 3rd dose was given on or after the 4th birthday)
3 Hepatitis B (3rd dose must be administered no earlier than 6 calendar months of age)
2 Hepatitis A (must be administered on or after the child's 2nd birthday and be at least 6 months
apart)
1 Varicella - chicken pox (must be administered on or after the 1st birthday or have a history of
having disease)
7th and 8th grade students will need:
2 MMR (both must be given on or after the 1st birthday)
5 DPT (unless the 4th shot was given after the 4th birthday)
4 Polio (unless the 3rd dose was given after the 4th birthday)
