Oklahoma Immunization Requirements for
2007-2008 School Year

Students enrolling in school must present verification that they have received the following immunizations:

Four Year Old Program
    1 MMR   (must be administered on or after the child's first birthday)
    4 DPT's
    3 Polio
    3 Hepatitis B   (3^rd dose must be administered no earlier than 6 calendar months of age)
    2 Hepatitis A   (must be administered on or after the child's 2^nd birthday and at least 6 months apart)
    1 Varicella-chicken pox   (must be administered on or after the 1^st birthday or have a history of having the disease)

Kindergarten through 9th grade students will need:
    2 MMR   (both must be given on or after the 1^st birthday)
    5 DPT's   (unless the 4^th shot was given on or after the 4^th birthday)
    4 Polio   (unless the 3^rd dose was given on or after the 4^th birthday)
    3 Hepatitis B   (3^rd dose must be administered no earlier than 6 calendar months of age)
    2 Hepatitis A   (must be administered on or after the child's 2^nd birthday and at least 6 months apart)
    1 Varicella-chicken pox   (must be administered on or after the 1^st birthday or have a history of having the disease)
