Oklahoma Immunization Requirements for 2012-2013 School Year |

| Students enrolled in school must present verification that they have received the following immunizations. |
CNP Schools Self Administered ASTHMA Medication Policy and Consent Form |

Pre-Kindergarten students will need: - 1 MMR - 4 DTaP - 3 Polio - 3 Hepatitis B - 2 Hepatitis A - 1 Varicella |

Kindergarten, 1st thru
12th grade students will need: - 2 MMR - 5 DPT/DTaP (unless the 4th dose was given on or after the 4th birthday) - 4 Polio (unless the 3rd dose was given on or after the 4th birthday) - 3 Hepatitis B - 2 Hepatitis A - 1 Varicella |

& 8th grade students will need an additional: - 1 Tdap booster |
